Medicals for New Applicants

New Applicants are required to undergo a medical which meets 'Group 2' standards with the Councils current medical provider.

The Council must be satisfied that you meet the 'Group 2' standards.

A medical to 'Group 2' standards will be required on first application and then every five years from the age of 45. On reaching the age of 65 a medical will be required annually.

The Council may also request a medical at any other time it feels may be necessary to ascertain a persons’ fitness to drive a licensed vehicle. The Council’s current medical provider is:

You are required to contact Fit 2 Drive Medicals and make an appointment for your medical examination, this is to ensure that the appointment time is convenient to you. Payment will be taken at the time of booking.

Cancellations at short notice or no-shows will incur a charge. If you need to cancel or rearrange an appointment you are required to contact Fit 2 Drive Medicals . Fit 2 Drive Medicals Terms and Conditions can be viewed here:

You will need to take the following with you to any medical:

  1. Bassetlaw District Council Medical Certificate
  2. Medical History Summary sheet from your GP
  3. If you require glasses and/or contact lenses for driving, you must provide a recent prescription from their Optician/Optometrist.

You must also:

  • Arrive at least 20 minutes in advance of your appointment time and you will be required to complete a Fit 2 Drive Medical Questionnaire.
  • Arrive with a full bladder, as a urine sample must be produced at the time of attendance and not before.

You will require the following details to complete the form.

  • Details of your GP/Consultant's details i.e. name, surgery and contact number.
  • Details of any medication that they may be taking. This includes name and dosage details

Failure to provide these documents may result in the medical form not being completed, and the cancellation policy will be implemented.
