Guidance Notes for completion of
application form
Please read this document before you
complete application form
As they get older, many people feel
they are not as steady on their feet as they were, and worry about
the risk of having a fall at home. Minor adaptations in the home
can often help to improve self-confidence, make you feel safer, and
give peace of mind to families.
PAS can supply:
- Grab handles
- Stair / hand rails
- Half steps
To qualify for the scheme:
- You should be aged 60 or over
- Live within the boundaries of
Bassetlaw District Council
The following conditions apply:
- There is a flat charge of £15 for
the work
- You can be a private home-owner,
council, housing association or private tenant
The Application Form
The following provides an
explanation for each of the different sections of the
application.
Someone else, such as a neighbour,
relative, or a health professional, may complete your form, on your
behalf. If you have any questions regarding your
application, please do not hesitate to telephone the PAS
office on 01909 535128. We can complete an application over the
telephone if you prefer.
Personal Details
We will need to contact you, so
please provide as much information as possible. You can tell us
things like: you are hard of hearing, or might take longer to come
to the door, you would prefer us to contact your carer, the route
to your home if it is difficult to find etc.
Your home and tenure
We ask what type of property you
live in, so we can assess the level of difficulty you may be
having. Also, whether there will be any problems fitting
adaptations because of building fabric or shared access, for
instance. If you do not own your home we may need to contact your
landlord for permission to fit any adaptations.
Ethnicity
We are required to collect this
information to monitor that our services are accessible to everyone
in the community.
Needs assessment
We ask questions about difficulties
you have getting around your home so that we give you the right
advice and adaptations, if appropriate.
Adaptations
Please tell us what work is needed,
e.g. grab rail by door, stair rail on the right side going up.
If this is a professional referral
please include measurements and mark the position if possible.
Details of referrer
If you are referring someone else
to this service, please ask their permission first, and please
ensure we have your full contact details. If this is a professional
referral, please include your job position and work address.
Service Description
Please read this document before you

Below is a description of some adaptations that
can be fitted through the Preventative Adaptation Service
(PAS).
Grab handle
A plastic coated metal ‘D’ – shaped
handle. Typically fitted: next to an external door to help
you get in and out more safely, by the toilet to help you get on
and off more comfortably etc.
Standard lengths are 12" (300mm),
17" (450mm), 24" (600mm).
Stair rail
Timber stair rail mounted on
brackets on to your wall. It will be left plain and sanded, or you
can varnish or paint it yourself at a later date. Stair rails can
be used internally and externally. External metal railing may be
available, e.g. a rail over awkward steps into the garden.
Half step
An external graduated smaller step,
this really is an ‘in-between step’. Fitted by an external door
where the height of the existing step is too high to manage
comfortably or safely, a half step can make every day life easier.
(Please consider whether a half step would be a tripping hazard for
anyone who needs to pass by your door).

Should I get advice from an Occupational Therapist
rather than apply to this scheme?
In certain cases you should contact
an Occupational Therapist at your local Social Services office, to
get an assessment, rather than apply to this scheme. Such example
cases are: if you are a permanent wheelchair user, or have a
significant illness or disability. Please telephone this office if
unsure.
What will happen next?
We will check your details to
ensure we can do the work, and we may need to contact you for
further information. We will then give your contact details to the
contractor who will ring you to arrange a convenient date to fit
the adaptation(s). To ensure your security we may need to give you
a password to use when the contractor comes to your home. We will
let you know about this if it is necessary and we will notify you
in writing of the contractor’s details.
Remember: There is
a flat charge of £15 for all adaptations. The
charge is the same whether it is just for a single item, or
a package of works. The contractor will collect this £15
charge after the work is done. You may pay in cash or by cheque –
the contractor will give you a receipt.
All the contractors we use are very
experienced in this kind of work. We trust you will find the
service beneficial.
