Help with PDF documents

You can download Portable Document Format (PDF) documents using Adobe Reader – available free of charge.

If your screen reader software (for example an audio-enabled web browser) is not compatible with Adobe Reader, Adobe provide a free online tool to convert the content of PDF files to text – so they can be understood by most screen reader applications.

CC1 Change of Circumstances form - Part 1 of a change of circumstances form. Part 1 plus one or more of the following supplementary pages must be completed, depending on the change.

Change of Circumstances leaflet This tells you which changes you need to tell us about and who to contact.

CC2 moving house form - The supplementary form to notify us of moving house. Please ensure you fill in a CC1 above as well.

CC3 change of rent form - The supplementary form to notify us about changes to your rent (private tenants only). Please ensure you fill in a CC1 above as well.

CC4 Income change form - The supplementary form to notify us about changes to your income or capital.Please ensure you fill in a CC1 above as well.

CC5 household changes form - The supplementary form to notify us if anyone moves in or out of your home or to claim single occupier discount on your Council Tax. Please ensure you fill in a CC1 above as well.

CC6 Death notification form - Notify us about someone that has died. Please note, you will have to print off a copy for each department shown at the top of each form. Please ensure you fill in a CC1 above as well.
