NottsInfoScript is an information portal for people with long term health and social care needs. It provides information on conditions plus wider areas that may be impacted upon e.g. benefits, support groups, leisure, work etc. It is intended to be used by health and social care staff, patients and carers to enable people to get the information they need when they need it most. Information prescriptions have been shown to increase self-care and involvement in care planning. In turn this can reduce hospital admissions and GP visits.
